Using the unbalanced insurance count, ten = -2, others = +1 I get:

mu = -0.034137
index = 1.409888as you said, but using the EORs:
tc (E) = 0.034706
tc (C) = -0.019608
dot (C, E) = 95.785882
dot (C, C) = 39.058824
b = 2.452349
mu = 0.082791
