Using the unbalanced insurance count, ten = -2, others = +1 I get:

tc (E) = -0.058824
tc (C) = -0.019608
dot (C, E) = 49.176471
dot (C, C) = 39.058824
b = 1.259036
mu = -0.034137
index = 1.409888

as you said, but using the EORs:

tc (E) = 0.034706
tc (C) = -0.019608
dot (C, E) = 95.785882
dot (C, C) = 39.058824
b = 2.452349
mu = 0.082791
index = -1.755517

Sincerely,
Cacarulo
